I recommend watching replays of games where you got a good team and reliving the moments when things went as you hoped, people cooperated, ward vision was good, supports rotated to help, and the enemy was on the run.
You know 40% of your games the other teeam is falling apart and reporting each other. You just need to refocus on those easy win games now and then to regain perspective and realize you are just as likely to win as lose the next game.
Remember that a team consists of 4 other people, and you.
You are part of the toxic team.
You can't deny that.
The reason you are anxious is because there is a dissonance between you thinking mmr is important and reflects how good a player you are and that you think you have no control over the result of matches - it isn't possible to reconcile them both - either accept that mmr is just a number and stop thinking it defines you or accept that it does and that if you want a high mmr that you deserve then you need to man up and git gud.
Either way, clinging on to a high mmr that you can't maintain is ludicrous.

Simple logic:
If you are better than your teammates (you are not) you will win more games than you lose, because on your team there would be 4 retards plus you (the better one) and on the other team 5 retards (most of the time). Dota gives every player a number for his skill. If your number is too low for your actual skill, you should climb by time.
Since you do not climb and stay low, you are a baddy.
Thats the bad news.
The good news:
You can improve in no time! You have so many big things you can work on, that climbing mmr is really really easy.
1. Pick wise (heros that match your lineup or you are strong with; do not put too much effort in support below 3.5 k mmr)
2. Learn to farm (Lasthits/Denies/Flashfarming midgame)
3. Do not feed (Just do not)
4. Built proper Items (Dagon Spectre might be fun, but might lose you the game).
5. Adapt your skillbuild (there is no “right” skillbuild for every situation)
6. Watch what is going on (Check minimap and other lanes often; use your tp if enemies dive)
7. Make the right decisions (Push/Farm/Gank/Defend/Rosh when the time has come etc.)
8. ALWAYS CARRY A TP
9. Check your enemies Inventories.
10. Do not rage (lowers apm, causes you to get muted, and nobody will listen to you anyways; If someone rages mute him)
11. If you get tired by playing, you are hungry or angry, just stop playing.
To do this focus on one aspect a time to improve:
Watch your replays to see your mistakes; watch pro replays/good streams, to see how they do it.
If you do not want to get better and make the effort, play on in your lowpool. But do not blame the system.

I get matchmaking anxiety too, but it's just because I'm afraid of losing. Especially that precious MMR.
Actually, Even I do all of those things you listed(or improving at it), and I'm a 1K scrub. Telling these things to someone who has about 3000 games under their belt probably won't do as much good as telling them to someone of my level, because most of them don't. Not that I'm necessarily better than them at the game. I still probably belong in the 1K bracket because I'm semi-new. The climb is painful, but a learning experience.
Not, of course, that normal people aren't anxious about it. Ladder Anxiety is a common phenomenon.

Alot of of people in this forum have exceptional knowledge of this game (mastery level theorycrafting) but can't execute properly when it comes to gameplay.
Anxiety in ranked match making is due to hitting a milestone and losing 200 mmr after 2 days. Aint as bad as sc2 1v1 ranked though.
The only reason why you don't want to play solo ranked is because you feel like you're not even worth your solo rating. Many people calibrate and then don't play solo because they know they don't deserve it.
Man up, play solo, deserve ur rating.
from 4 k mmr it gets harder and harder. Every 200 points higher u earn u must make less mistakes and play better, be more unpredictable and give your enemies less hope for the win, rotate lanes, stack more camps, use smoke more often, deward, ward in less obvious places and so on.
Skill gaps between brackets like 3k-4 k and 4k-5k are huge. So everytime u gain for example 200 mmr and hold this mmr for a couple of days, u may say that u have improved a bit. But when u lose a lot of mmr, means that u dont belong in this bracket.
I think everyone of us have experience with toxic teams or leavers or intentional feeders. Well with feeders u cant do shit, but u can have an impact on flaming teammates by good play (if u play core) or trying to calm them down. If they see that u are starting to carry the game, make no mistakes - they follow u and quit shit talking.

Don't be scared of losing rating, as much chance of winning it as losing. If you struggle to win learn new heroes, or mix your shit up.
If you lose games, don't just keep grinding, get something new going or be happy to accept your loses. Simple thing is to look at your heroes this patch and see which are your bread winners, and if so why? Learn to understand your own play style and where you excel and where you fall over.
I used to have this with csgo, was stuck in silver always thought that my rank mattered, eventually i took a break for 2 months because i got scared and didint want to lose rank. Eventually i came back, i no longer cared about my rank and played for fun, i managed to rank up 3 times in 3 days.

This helps you to focus on your gameplay more and disregard your teammates
Sure you might miss few important notifications, but you will also learn to be more aware of your map.
Honestly it was the best decision i have ever made, matchmaking has been so much more enjoyable (i dont mute when i play 4 or 5 stack as i have enough people to back me up when the toxic teammate starts to play the blame game)

There are 4 other players on your team who might feed and throw the game. If they do, its very hard to win. You can only control what you do.
My last game was frustrating because there was about a 2 sec delay for 10 minutes that made it hard to save feeders.

The huskar started the game saying "I can't win today. My team always sucks." Then he proceeded to random Huskar, a hero on which he averages a 1.1 KDA. I protected him early, got him FB gold and total free farm. He used it to get Hand of Midas and farm/feed accomplishing nothing the whole game. He did not achieve even his terrible average 1.1 KDA and ended up 4-14-6.
Sometimes you just can't win a game. But just as often you are assured the win because of the other team feeding. It all balances out. A lot of your games are unwinable, its true (about 40%) But more than half of them really are winnable and if you are significantly underated you really could win even most of the "unwinable" games.
a totally unscientific estimate but i think that every 500 you are below your true mmr is worth 10% wr. i.e. 500 below=60%, 1000=70%, 1500=80%, 2000=90%
The people who get anxiety get it because they know they can't maintain their mmr they desperately want for some sad attempt to gain some self worth

I think I am scared of solo ranked match making. Because everytime I play it I will get an unwinnable, toxic team and it also becomes impossible/really hard to get that lost mmr back. It wasn't like back in the days. Now you'll get smurf try-hards teams.
