They just come off MMR, most likely the average of the players involved. Seems to be something like <3.5k = Normal, <4.5k = High, >=4.5k = Very High. You used to be able to filter the in-game match list to them, was an indirect way to check MMR.

When you see a lit of matches on Dotabuff, they will sometimes be subfixed with "Normal Skill", "High Skill" or "Very High Skill", but a lot of the time they will have no subfix at all. How come?
